Cáncer hepatocelular y su relación con el virus de la Hepatitis C, desarrollo de la enfermedad y diagnóstico
Resumen
El carcinoma hepatocelular (CHC) es el tipo más frecuente de cáncer primario de hígado y está estrechamente relacionado con la infección crónica por el virus de la hepatitis C (VHC). Este virus, de la familia Flaviviridae, se transmite principalmente por vía sanguínea y puede generar hepatitis crónica, cirrosis y, en fases avanzadas, CHC. La prevalencia mundial del VHC se estima entre 2 % y 3 % de la población, con variaciones regionales. En Ecuador, se han registrado altas tasas de infección, especialmente en provincias como Manabí y Esmeraldas.
La progresión a CHC ocurre principalmente por inflamación crónica y fibrosis, que inducen cambios genéticos y moleculares en los hepatocitos. Entre las complicaciones clínicas destacan cirrosis, insuficiencia hepática, trombosis, diabetes tipo 2 y trastornos neuropsiquiátricos.
El diagnóstico temprano es clave e incluye pruebas serológicas, PCR, ecografía, tomografía, resonancia, elastografía y, en casos necesarios, biopsia. El uso de antivirales de acción directa ha reducido significativamente la progresión a cirrosis y CHC. La vigilancia activa, la detección precoz y el acceso a tratamiento eficaz son esenciales para disminuir la mortalidad y mejorar la calidad de vida de los pacientes.
